PERANCANGAN ULANG MAP REKAM MEDIS SESUAI DENGAN STANDAR DAN KEBUTUHAN PETUGAS REKAM MEDIS DI PUSKESMAS DAU KABUPATEN MALANG

Abstract

The patient's medical record folder is "a part of the cover or wrapping that is used as a protector of the folder in order to protect medical forms or patient history sheets from the time the patient is treated until he is declared cured by the medical staff who provide services in it so that the form is not scattered, not damaged, torn, folded and so on This service uses the design method through the corel draw application. Data collection methods used are observation, interviews, and documentation. The results of the data obtained are used to find out what aspects need to be re-designed on the medical record folder at the Dau Health Center, Malang Regency. The results of community service obtained a number of three proposals for the design of the medical record folder. Of the three proposed medical record folder designs that had been submitted to the Dau Health Center medical records officers, one design was selected. The selection of one design from the three designs is carried out by means of FGD (Focus Group Discussion). The strengths of the proposed medical record folder design that were selected included a title or introduction, there were three medical record columns on two sides, there was a filling column for the year of the last visit, and there was a picture of the Dau Health Center on the front cover.
