The application of information technology in the health sector has become a crucial need to improve the efficiency and quality of medical services, including medical record management. Nur Hidayah Rengel Clinic, located in Tuban Regency, still uses manual recording methods. This condition results in service delays, incomplete data, and low staff efficiency. This community service activity aims to implement a web-based outpatient electronic medical record information system, enhance staff capacity in operating the system, and provide supporting documents in the form of technical guidelines and user manuals. The implementation method includes preparation, technical training, document development, monitoring and evaluation, and project closure. The results showed that clinic staff were able to operate the system well after training, and the system was successfully implemented for outpatient services. The technical guideline document also serves as a practical reference for system operation and maintenance. In conclusion, this activity not only succeeded in transforming manual recording into digital form but also improved the clinic’s human resource capacity in managing health data independently and sustainably.
